Hi ladies! Important message: If you had a sports physical from a healthcare professional last school year 2019-2020, you do not need to get one this year.

If a student athlete had a physical completed in 2019-2020 school year they are only required to fill out Form B for the upcoming school year 2020-2021. Form B is filled out by the student’s parents or legal guardians. If they did not have a physical performed by a health care provider last year (2019-2020) they will need to do the complete physical.

You may get a Form B from me or print your own from the NDHSAA website at this link.
